The Minister for Justice has been pleased to appoint the person holding the post of Registrar-General to be Registrar-General of Births and Deaths.
G.N. No. 8 of 1943
Every Assistant Registrar-General and every Assistant Registrar is hereby appointed to be an Assistant Registrar-General of Births and Deaths.
DIRECTIONS
THE BIRTHS AND DEATHS REGISTRATION (DUTIES OF DISTRICT REGISTRARS) DIRECTION
G.N. No. 106 of 1969
1. This Direction may be cited as the Births and Deaths Registration (Duties of District Registrars) Direction.
2. It is hereby directed that the duties of a District Registrar in a district shall, in the absence or during the illness or incapacity of the Administrative Officer in charge of a district, be performed by the Administrative Officer stationed at the headquarters of that district who is next in seniority.

1. This Direction may be cited as the Births and Deaths (Registration Officers) Direction.
2. It is hereby directed that the duties of the District Registrar of Births and Deaths in respect of the Moshi Rural, Bagamoyo, and Morogoro Urban Districts shall be performed by Registration Officers assigned in writing in that behalf by the Registrar-General of Births and Deaths.
3. For the avoidance of doubt, the provisions of the Appointment of District Registrar of Births and Deaths Notice, shall not apply in respect of Moshi Rural, Bagamoyo, and Morogoro Urban Districts *.

1. This Direction may be cited as the Births and Deaths (Registration Officers) Direction, and shall be deemed to have come into operation on the first day of May, 1985, in this Direction referred to as "the effective date".
2. It is hereby directed that with effect from the effective date, the duties of District Registrar of Births and Deaths in respect of Hai District and Kilosa District shall be performed by the respective Registration Officers assigned in writing in that behalf.
NOTICES
G.N. No. 40 of 1981
1. This Notice may be cited as the Appointment of District Registrars of Births and Deaths Notice.
2. Every public officer who is the Manpower Management Officer for a district or a sub-district is hereby appointed to be the District Registrar of Births and Deaths for the District or sub-district for which he is the Manpower Management Officer.
3. The Appointment of District Registrars of Births and Deaths Notice, 1978, is hereby cancelled.

1. This Notice may be cited as the District Registrar of Births and Deaths (Appointment) Notice.
2. The Registrar-General of Births and Deaths is hereby appointed District Registrar for the District of Ilala with effect from 1st July, 1981.
3. The appointment of the Registrar-General of Births and Deaths as District Registrar for the District of Dar es Salaam is hereby cancelled.
4. For the avoidance of doubt, the provisions of the Appointment of District Registrar of Births and Deaths Notice * shall not apply in respect of Ilala District.

1. This Order may be cited as the Births and Deaths (Compulsory Registration) Order.
2. The provisions of the Births and Deaths Registration Act relating to compulsory registration of births and deaths are hereby extended to the birth of every child born alive and the death of person dying within the limits of the city or township specified in the Schedule to this Order.
3. The provisions of this Order shall not affect the provision of the Births (Non-Native Compulsory Registration) Order, 1949.
4. The Births and Deaths Registration (Tanga Township) Order, 1952 and the Births and Deaths Registration (Arusha Township) Order, 1959 are hereby revoked.

1. These Rules may be cited as the Registration of Births and Deaths Rules.
2. Births and deaths may be registered during office hours at the office of the district registrar of the district in which the birth or death took place.
3. (1) Any person, whose duty it is to register a birth or death, may, instead of attending personally, apply to the district registrar of the district in writing to register the birth or death in manner hereby provided in the following cases *–
(a) if resident more than twenty-five kilometres from the district registrar's office; or
(b) if unable through ill-health to attend personally.
(2) The district registrar, on receiving such application, if satisfied that personal attendance should be excused, shall send to the applicant the appropriate form which the applicant shall fill up, sign, and return to the district registrar. The district registrar shall, on receipt of such form, copy the particulars contained therein into the register and shall forward the form to the Registrar-General as an annexure to the quarterly return rendered under section 22 of the Births and Deaths Registration Act *. The entry of the particulars into the register shall be deemed to be the registration of such birth or death.
(3) Notwithstanding anything contained in subrule (1) or subrule (2), where a child is born or a person dies in a hospital, maternity home, nursing home or clinic, the person whose duty it is to register the birth or death may, instead of attending personally or applying to the district registrar under subrule (1), fill up and sign the appropriate form and after having it counter-signed by the medical officer who was in attendance at the time of the delivery of the child or during the last illness of the deceased person, forward it to the district registrar. On receiving such form the district registrar shall make entries in the register and forward the form to the Registrar-General in the same manner as in the case of a form received by him under subrule (2) and the entries so made shall be deemed to be the registration of the birth or death.
4. The registers in the custody of a district registrar and the registers, copies of registers, returns, and indexes in the custody of the Registrar-General may be inspected, on payment of the prescribed fee, during office hours.
5. Any birth or death occurring on board any ship while within territorial waters shall be registered by the person whose duty it is to register it at the nearest district registrar's office to the port in which the ship is lying or to the next port of call.
6. The following fees 1 shall be chargeable * –
(1) For the registration of the birth of any child where such birth occurs in any area prior to the date of the extension of compulsory registration to such area by order under the provisions of section 27– |
(i) Registration of the birth of any child for any period up to 3 months. |
(ii) For any period in excess of 3 months up to 10 years. |
(iii) For any period in excess of 10 years from the date of such birth. |
(iv) Registration of the death of any person for any period in excess of 1 month. |
(v) Registration of the death of any person for any period in excess of 10 years. |
(2) For the registration of the name or alteration in the name of any child whose birth has been previously registered. |
(3) For the inspection of any registry copy of a register, return or index, for each entry inspected. |
(4) For a certified copy of any entry in the Registrar-General Certificate. |
7. [Revokes the Registration of Births and Deaths (Amendment) Rules *.]
8. The forms in the Schedule hereto shall be used.

1. These Rules may be cited as the Birth Certificate (Short Form) Rules.
2. The Registrar-General or any district registrar in whose custody is any register of births may on the application of any interested person issue a certificate in the form set out in the Schedule hereto (hereinafter called a "short form of birth certificate") in respect of any person whose birth is entered in such register (hereinafter called "the registered person").
3. In compiling a short form of birth certificate, the Registrar-General or district registrar, as the case may be, shall insert opposite the word "name"–
(a) the personal or Christian name or names of the registered person as entered in the register of births; and
(b) where it appears from the entry in the register of births that the registered person is legitimate or where it is proved to the satisfaction of the Registrar-General that the registered person was subsequently legitimated, the surname, if any, or name of his father; or
(c) where it appears from such entry that the registered person was born out of wedlock, the surname, if any, or name of his mother:
Provided that where the entry in the register of births is marked with the word "Adopted" or "Re-adopted", or with the words "Adopted (Kenya)", "Adopted (Uganda)", "Adopted (Zanzibar)", "Re-adopted (Kenya)", "Re-adopted (Uganda)", "Re-adopted (Zanzibar)" and the court which made the adoption order has specified a name or surname which the registered person is to bear instead of the original, then such name or surname shall be inserted in the short form of birth certificate in place of the name or surname required by this rule to be inserted.
4. The fee for the issue of short form of birth certificate shall be one shilling.
5. The fee for any search shall be two shillings.
